Prior to your first appointment, please review the following materials:
- Medical history/family history form
Prior to your first appointment, please take a moment to fill out this medical history form and bring it with you. It will help guide the care team.
- Records requests
Please bring any past medical forms or records related to your heart health to your appointment.
- Insurance information
Many insurance plans cover genetic testing and heart and vascular screening so they are affordable for families. Please bring your insurance card to your appointment.
What to expect during your appointment
When you first visit, we will review your medical history and draw a detailed family tree to learn about you and your family’s risk of having a hereditary heart condition.
We will discuss how genetic testing or clinical screening may be helpful for you and your family. In some cases, a genetic test requiring a small blood sample can help determine your risk for a particular condition. In other cases, a clinical heart and vascular screening, such as an echocardiogram, is the best tool.
Post-appointment follow-up process
In a follow-up visit, we’ll help you understand your test results and identify other family members who may benefit from genetic testing and/or heart and vascular screening.
Living with an inherited heart condition can be emotionally challenging. Our team of experts can help with many of the personal aspects of having an inherited condition, including helping you share information with relatives and make decisions about reproductive options.
